Rules Governing Nomination

PROCEDURES FOR NOMINATION

For this year, the nominator may either nominate on-line or through a hard-copy nomination form. They may NOT use both at the same time for one nominee.

 

  1. ON-LINE NOMINATIONS

    Those who choose to nominate on-line can simply log-on to the official TOFIL website at www.tofil.ph, click the “NOMINATION” applet and select “on-line” from the drop down menu. Nominators will be required to register on-line before they can proceed to fill up the on-line nomination form and corresponding supporting documentation. A filled-up “Nominee's Certificate” in scanned file must also be uploaded into the system in order to proceed with the nomination proper.

    Nominators only need to follow the instructions given by the on-line system. No additional hard copies of documents are necessary, unless requested by the TOFIL committee. All entries (on-line Executive Summary, on-line Nomination Form, on-line Nomination Book) must be filled up by the nominator before the system can validate the nomination. All supporting documents must be in digital form (i.e. scanned, digitized or in soft copy files) and uploaded into the nomination system. The system is already configured to limit the number of text and image files entered to conform to each nomination's prescribed length and font sizes.

    Nominators have the option to nominate as many nominees as they wish and may edit their entries for as often as they can until the deadline of the nomination period has lapsed.

    The TOFIL Committee reserves the right to request for hard copy documents from the nominator to validate images uploaded during the screening process, should they deem necessary to do so.

  2. HARD COPY NOMINATIONS

    Those who choose to submit hard-copy nominations must submit the following documents:

    1. An EXECUTIVE SUMMARY following the prescribed format should not exceed two pages and text should be typed using a minimum font size of 12 points. The executive summary should be the first pages of the nomination book.

      Format of the Executive Summary:
      1. Personal Profile
        1. Educational Attainment
        2. Character, Integrity and Moral values
      2. Impact on Public Welfare and National Development
        1. Socio-economic and/or cultural impact
        2. Geographic Impact
        3. Years of dedicated work and service
      3. Participation in government, professional, socio-civic, religious organizations and non-governmental organizations
        1. Level of participation
        2. Length of involvement
      4. Awards and recognition
      5. Impact on field of endeavor or professional field


    2. A NOMINATION BOOK* which should not exceed 40 pages (exclusive of the Executive Summary). Text should be typed using a minimum font size of 12 points.

    3. ATTACHMENTS** to the nomination book should not exceed five multiple-pages, and each multiple page should not exceed 20 pages.

    4. IV. One (1) original copy and five (5) photocopies of the ACCOMPLISHED NOMINATION FORM AND NOMINATION BOOK AND ATTACHMENTS THEREOF, together with six (6) 2"x2" COLORED PHOTOS of the nominee.

      Note: The Nominator must cite in the nomination book* the nominee's background, career, range of work, scope of activity and exceptional achievements. Quotations, statements of authorities, honors and awards may be included in separate sheets as attachments**. More importantly, the nominator must present how the nominee's accomplishments have benefited humanity in general and the nation in particular.

    5. One (1) compact disk (CD) or DVD containing scanned copies of each page of the nomination form, executive summary, and nomination book.

The nomination covers accomplishments up to June 30, 2011. Nomination should be made on the prescribed TOFIL Award Nomination form. Additional copies are available at all JCI Senate Philippines Chapters and all The Insular Life Assurance Co., Ltd. district offices nationwide.

NOMINEES SHOULD BE NOMINATED UNDER ONE CATEGORY ONLY

The accomplished nomination form and all supporting documents must be submitted on or before August 31, 2011 to:
